Document in Latin, Tagliacozzi's libellus supplicatorium or formal request for admission to the College of Medicine at Bologna, given at Bologna, 'in sacrario Eccl[es]ie Chatedralis [sic] antiquo', 18 December 1576, approved and endorsed by Ovidio Giubetti as prior of the College of Medicine and Gabriele Beato as prior of the College of the Arts, and with a further endorsement by the notary to the colleges, Giovanni Battista Canonico, recording Tagliacozzi's acceptance ('D. Gaspar gratias Egit'), one page, oblong 4to (220 x 280mm) (losses from ink acidification, minor spotting). Provenance: Sotheby's, 18 May 1989, lot 341.

'Suplicatur humil[ite]r et devote pro parte et nomine ... Gasparis dudum Ioannis andree taglicotie ... [It is humbly and devoutly requested on behalf and in the name of ... Gaspare son of Giovanni Andrea Tagliacozzi ...]': having satisfied the requirement of being a citizen of Bologna by birth, and having graduated as a doctor of medicine, it is now requested that Tagliacozzi be enrolled in the College of Medicine at Bologna and be granted the first vacant place in that college.

Tagliacozzi had applied for admission to the College of Medicine in May 1576 and was accepted on 29 November (which was, irregularly, a little before he received his doctorate on 5 December). The present libellus supplicatorium formed part of the final ceremony of his admission on 18 December in the Old Sacristy of the cathedral of S. Pietro. According to ABPC-online/Americana Exchange, NO OTHER DOCUMENT BY OR RELATING TO TAGLIACOZZI HAS APPEARED AT INTERNATIONAL AUCTION IN THE PAST 40 YEARS.
